License Renewal Form
To utilize this form you must have been issued a CARS National or Regional license after Jan. 1 2010. You also need to enter your e-mail address that you utilized during your last correspondance with the CARS office (for verification purposes). Dont worry if the e-mail is no longer active, you can ask for the email to be changed in the feedback/comments field at the bottom of the form. But you will still need to enter it as you did during your previous license application. If you do not remember this email you will need to contact the CARS office directly.

A signed CARS Waiver must be received for your submission to be approved. Please upload a signed copy of the CARS Waiver which can be found here: License Application. You can alternatively FAX the waiver to the CARS Office. If you are not changing any information, we will only require the signature and payment to process your application.
